The only downside I see is that there are times when drawing someone out of their gloomy, self-focused thinking is actually more beneficial than talking to them about their problems.
Of course, it's never an all-or-nothing kind of thing.

Not to be a party pooper but I try not to ask that question unless I really want to know the answer. I asked a cashier at a convenience store the question one time and was there for a good five minutes listening to her woes. They basically amounted to "my mom pissed me off and ruined my life for today."
I learned my lesson.

Good point. I do say "How are you doing" a lot. For me, it gives that person a chance to let something out of they need to. Like that cashier.
I guess my point is to be cognitive of how you greet someone, which you learned that it is important.
